I used these guys with satisfaction for work: LogoUp.com Custom Embroidered Shirts & Hats (http://www.logoup.com/)

You have to find when they are running a special to input/digitize your logo for free, otherwise $50 extra. They also run many specials on the various shirts/gear and will run off any number of items once the logo is in their system.
