You lost the neutral (return path) if you had a short it would have tripped the main. Basically had either of the hot legs came in contact with the ground or neutral it would have shorted and popped...

You say its custom but its an XSRA, A custom would be XRA 1322. The "S" means stock, like redtops inventory. Are you sure it came from CMS?
