If you're really lucky, the fish will push the barb all the way thru ya with a head shake
Then all ya gotta do is move your hand in tune with the thrashing of the fish while you fumble for yer pliers.
Here's a tip---if you're right handed, wear your cutting pliers on the right side of your belt.
And yes, I'm talking from experience here
