One common live-well leak not mentioned yet is you can have a leak in the plumbing on the output side of the pump. Similarly water systems tend to do this.
Also common are water outlets that can allow water in when the boat rocks. A good practice is to have any such bilge pump hoses loop up as high as possible, then connected to the fitting, but often they are cut as short as possible and it leaves a perfect slope for water to run down into the bilge when the boat rocks.
