i had the same problem twice. supposedly, its been resolved on newer bags, but i switched to the vs so I don't know.

the straps on the belt bags get pretty torn up too, i wrap them in duct tape where they come in contact with the belt and that seams to get rid of that issue

I keep a pin with a handle in the bag now so when they get caught I stick the pin in with the hook and make room for the barb to back out. I wish I carried this in the past. It works a lot better than the method they use in the emergency room : )
