The reports that I have read on this accident idicate that the explosion and fire began when the "unnamed Captain turned on the switch for the blowers".
The old school teaches us to have no one aboard during fueling operations and that all hatches be closed. Upon finishing refueling the Capt. should open the cabin and "sniff" the bilges for fuel odors and fumes. Finding no fumes the Captain shall continue to open hatches and vents and turn on the "Bilge Blowers" system to clear the air.
Apparently in the incident reported the crew were still on the vessel and she was not closed up during fuel operations.
Sad but accidents do happen. Most often with cause.
