"Pullin' the hook"

So, how many times have you had a nice one "right there" then "ping", and goodbye fishie. Was it from "horsing" at the last minute, bad hookup (hooked by a "hair"), too tight a drag, to small (or large) a hook, giving the fish a little slack and the hook falls out ?
Everone has stories about the one that got away. So whats your story and what have you done differently since it happened ? Sharper hooks,
less drag, "softer" rod, change to braid (or change back to mono), play out the fish longer with less drag pressure ? Any tips to reduce fish loss ?

Maybe there are no clear cut answers.
