Very sad. My sincerest thoughts and prayers go out to him and his family.

Years ago, a small crew of us used to fly fish the mouth of the Merrimack in Newburyport. The bottom is very fluid sand. Orvis used to make these manually inflatable PFD that had pockets for 4 large fly boxes. It was as standard gear as my waders. I have to admit, we put ourselves in some pretty dangerous circumstances. Eventually we stopped wearing the PFDs. That was our mistake. I am by no means even suggesting that this man made any mistakes. Just sharing my own experience.

Next time I don the waders, I shall revisit my korker studs and my PFD - especially on the breachways. A few surge waves into those rocks, even with others right there, could be VERY dangerous.
