You could just have any generic system that submits all the info into an e-mail and sends it to you (which im sure is what they're doing).

Just a website supporting php or asp would be the easiest.

In laymans terms, its just a form that sends you an e-mail, no special software necessary.

Would take about 5min to make.

ms FRONTPAGE can do that stuff easily if you got access to a host that supports frontpage extensions, which alot do now adays.
