Well, I use the middle attachment for tandem hook rigging our 14" Real-Eels and the front loop as the line tie.

the jig head goes on with the hook going through the back.. on my eel, the back is the flat side. I dont think you need a 2nd hook with my 8.5 inch eel, but on my 12 inch eel which i am about to release you should rig a rear hook- a 6/0 or 7/0 live bait hook would be best and with that hook you want it to stick out of the belly- this will act like a keel. So the front hook us up and the rear hook is down...When you slide the jighead on, make sure to use some zapagap to keep it all together- alot of casting may rip the rubber and the eel may slide around if you dont use glue.
oh- like canalman says, the rear facing loop is to tie off the leader-

Besides using the eyelet in the middle of the Wobblehead to tie the second hook off to , ...if you want the lure to dive deeper , tie your line or leader to the middle eyelet and it will go deeper.

nice Zeno
another thing i do is to tie a few half hitches around the the base of the wobblehead and glue it up before i tie off on the middle loop- this will help prevent tearing- if you dont do that and you get a rear hookup, its definately going to destroy the plastic.
