I fell off a jetty one time and when I got up,lo and behold a large popper was stuck to my shoulders,I tried to pull it out but the barb was in deep,so I went to the local hospital and they used the push through method but when they pushed it through they had nothing to cut the barb off with,so I rummaged through my tackle and came up with pliers,lucky they gave me a pain killing shot first because they had a hard time cutting the barb but eventially they cut it off and pulled it through.

When I was a kid, I had a redfin get stuck in my right thum, right into the joint. Went to the hospital and they worked on it for around 1/2 hour. They ended up cutting my finger open more to get the barb free. No way to push it out, it was right in the joint. MAN that hurt
