1. Don't use 'em. normal NN work fine for me.

2. I use 5/32. I get almost no misdrilled blanks maybe 1-2 a year, even w/less than sharp bits 1/32 makes big difference in the stiffness of longer bits. Plus I seal w/epoxy and the bit of extra space works out well.

3. I rough them up a bit and epoxy, although sometimes they still work free after being assaulted by rocks.

I still prefer a screw driver in a vise for tail wraps although I have a couple pair of the rounded needle nose pliers.

5/32" bit all the way through for me.

I think all lips will come loose after a while with or without epoxy. Although if you can get a little epoxy in the lip slot before you push in the lip and give it a tight tail wrap it should be fine
