Shouldn't ever happen and I have to shake my head when the news reports that two "seasoned" captains were at the helm. Seasoned might mean two old farts napping at the helm. Even if it were on autopilot, how does it put the ferry into the middle of the jetty, it wasn't like they missed the entrance by a boat length.
Back when I started striper fishing out of Scituate harbor, I was in my 17 foot basstracker Vhaul and had an old Interphase monochrome fish finder with nautical mile log I relied on for navigation. My weekend one day to fish meant I'm going if the seas allowed and many days the biggest challenge was getting clear of the harbor in pea soup fog. Once clear I'd have a milk run of ledges to hit between there and Cohasset and it was all on nautical miles, compass heading and shut down the outboard when getting close to listen for the waves hitting the ledges. Once close I'd idle in until I could see the surf on the ledge and then the trolling motor went down and the fishing began. The return trip was the same and I never hit the harbor jetties .

Too early to tell in both situations. Likely people effed up but we don't know that for certain yet. Both will have investigations to determine culpability. Investigations may (or not) lead to changes that prevent future incidents.

We have been through this before Paul but what is it about fishing in pea soup fog without radar that is a good idea?

Chris, it wasn't foggy when I went at 3:00 AM. I was North of Conimicut Light and saw the bank moving up the bay so I headed for the barn. I made it fine. It's the other nitwits I have to worry about.
You are right though, I need to get radar. next year. Need to do some stuff in SC first.

If it was a crossing situation and you were hit on the starboard side you didn't have the right of way, it's your fault.

I talked to a friend of mine who works on the ferry, he happened to be in Woods Hole and not the high speed ferry that night. They hit the jetty at 42 knots, nearly full speed. Heads are going to roll...The pilot was operating the vessel but the Captain was standing beside him.

From what I have heard the navy ship was DIW, likely awaiting transit time, the other ship was transiting into port and for a reason not yet disclosed turned around mid channel and was heading back out when it stuck the Fitzgerald.
Sitting in this area waiting to set navigation detail until closer to 6am is not uncommon, since most port times are in the daylight at Yokosuka.
I have sat aboard ship[ many times in this area during my 6 years stationed in Yokosuka. The Fitzgerald would have had lights on designating them as DIW and not under power. If the supply ship turned around quick enough, the navy ship may not have had time to get out of her way.
I think there is something more to this going on that has not been released yet....

Interesting points. However DIW there along the entrances of for shipping into Tokyo would be a bit much, no? And if the merchant was on autopilot with nobody on bridge (something others have speculated)

It was pretty common when I was stationed there to "park" or "loiter" in that area, and wait closer to sunrise, and then set navigation detail and start our transit into Yokosuka navy base.
If she was not DIW, with CIC and the bridge minimally manned, I don't see any possible way this could happen. You always have at least several operation specialists in CIC watching the radar and plotting all traffic. Anything with a close intercept is sent to the bridge and placed on the plotting board. They have several topside lookouts at all times, plus a small group on the bridge.
20 years in the navy and I cannot fathom the entire bridge and CIC watch team screwing this up. I have a feeling we will hear more details soon... something fishy is going on... whether it be total incompetence of the Officer of the Deck on the bridge or extreme course and speed changes by the Filipino crew, it just doesn't add up. Both of these ships are over 500 feet long.... It sounds as if the Fitzgerald never even saw the danger.. no collision alarm... strange..

A screenshot provided by vessel-tracking service MarineTraffic shows the route of the container ship.... this does not look accidental to me....

All reports show the initial U turn was 30 minutes prior to the accident
